*  Only paid games will be stamped

 * Any age may enter – Not valid for league play

 * Bowling priced by time:  10 minutes equals 1 game

 * Special group rates not eligible

 * Employees not eligible to enter

 * Total strings in a group may be combined on 1 card

 * Only 1 name per 10 strings that are stamped can be entered in the drawing

 * Only 1 prize per person

 * Other restrictions may apply

DISCLAIMER MBA Frequent Bowler Rewards Program

1.        Who can enter?  This MBA Frequent Bowler Rewards Program is open to all MBA patrons and ballot entries received from all participating MBA Bowling Centers between the dates of January 11, 2010 and May 16, 2010. 

2.        Methods of Entry:  To enter the giveaway, you must go to any participating MBA Bowling Center and bowl at least 10 paid games to be eligible.

3.        Drawing:  A random drawing will be held on June 3, 2010 during the MBA Youth State Tournament Banquet from all eligible entries received to award the prizes.  Eligible winner will be notified by telephone and will have 48 hours to respond to the telephone notification or they forfeit their winnings and a new randomly drawn winner is chosen.

4.        Prizes:  1^st place – Dell Desktop Computer, 2^nd Prize – Apple iPod Touch, 3^rd Place Garmin GPS, 4^th place – 4 ball set of Candlepin Bowling Balls.  A person can win only one of the 4 prizes; if the same person is drawn for multiple prizes then another name will be drawn until a new name is picked.

5.        Any age may enter this contest.  Entries received become the sole property of Massachusetts Bowling Association.  The MBA is not responsible for technical issues which may prevent an entrant’s ability to participate in this Giveaway or by any human error which may occur in the processing of entries in this Giveaway or for typographical or other errors in the handling of the Giveaway materials, administration of the Giveaway or the announcement of the prizes.  The MBA reserves the right to cancel or modify the Giveaway if there is any suspected or actual evidence of fraud which may destroy the integrity of the Giveaway in its sole discretion or for any other reason.  Any fraudulent entry or entry that contains misinformation is null and void.  All decisions of the MBA related to the Giveaway are final. 

6.        By entering, entrants consent to allow the MBA to contact him or her via telephone and/or email to confirm that he/she has won any one of the 4 prizes.  Also by entering, entrants consent to the use of his or her name, city name, image and/or likeness for advertising or publicity purposes without additional compensation or permission, unless prohibited by law.  No substitutions, transfers or cash redemptions of prizes can be made.  Winners may be required to sign and return an affidavit of eligibility/release within 5 days of notification attempt or prize will be forfeited and alternate winner may be selected.

7.        Odds of being a winner depend on total number of eligible entries received.  Void where prohibited or restricted by law.  All federal, state and local laws apply.

8.        All contestants release and forever discharge the MBA and their respective employees, owners, partners, affiliates and subsidiaries (collectively “Affiliates”) from and against any and all liability and/or claims with the respect to or in any way arising from this Giveaway, participation in the Giveaway and acceptance or use of the prizes, including, without limitation, any liability for personal injury or property damage or loss.  Contestants agree to defend, indemnify and hold the Affiliates harmless from any or against any and all liability and/or claims with respect to or arising from contestant’s acts or omissions in connection with participation in the Giveaway and use of any prizes.
